Accessibility Needs Documentation Guidelines
To establish an accommodation plan, you will need to provide documentation of your accessibility needs from a licensed professional. Providers such as physicians, nurse practitioners, psychiatrists, or psychologists, are examples of licensed professionals. In some cases, an IEP or 504 Plan may be all that you need to document your accessibility needs.
Documentation Requirements
- A specific diagnosis
- The date of diagnosis or how long the provider has been treating you for the condition(s)
- A description of its current impact
- A description of the expected progression or stability of the condition
- Documentation should be on letterhead, signed and dated
