UCollege Forms

• Declaring an Exploratory/Undecided major in UCollege (PDF): You must meet with a UCollege Advisor to finalize the process.
• UCollege Petition for Exception (PDF): e.g. below admissions requirements, late enrollment, late withdrawal
• UCollege Provisional Admission Contract (PDF): This form is for provisional admission students which have become Ineligible to Enroll due to a past semester's academic performance.
• UCollege Reinstatement Petition (PDF): This form is for students which have become Ineligible to Enroll due to a past semester's academic performance. If you wish to enter University College or remain in University College, but do not meet the 2.0 semester grade point average and/or 2.0 cumulative grade point average, you must submit this form prior to the beginning of the semester.
