Flu shots are covered for faculty, staff, retirees and dependents (children 7 years and older accompanied by a parent or guardian) enrolled in a UM System medical plan. Registration must be completed for each individual receiving a flu shot. A confirmation email with a consent form will be sent for each appointment scheduled.
Preparing for your appointment
- Prior to your appointment, complete the consent form included with your confirmation email. Bring the consent form with you to your appointment.
- Make sure to have your employee ID (if applicable), insurance card and confirmation email (digital or print copy) handy when arriving to your appointment.
- Please arrive no more than five minutes before your scheduled appointment. This will help us keep vaccination spaces distanced.
